The City of Salem's Senior Center Committee has put together a survey to determine the public's interest for a new Senior Center/Community Life Center.  The survey is confidential.  Individauls may leave their name and address if they would like to be invited to discuss programming and a location for the new Center at a meeting later this summer or fall.  

The survey is in both English and Spanish and either version can be downloaded at the links below.  Once completed, individuals may drop the survey in drop boxes located at Salem City Hall, the Salem Council on Aging, Salem Public Library, Crosby's Market, Steve's Market or the Salem Housing Authority.  Or, you may mail the survey to the Council on Aging, 5 Broad Street, Salem, MA 01970, Attn: Senior Center Survey.  

The Senior Center Committee greatly appreciates your participation and looks forward to reviewing your thoughts and information.
